Action taken report (ATR)

An action taken report (ATR) records what management has done about each audit finding, who owns it and whether it is closed.

Why Action taken report matters to internal audit and controllers

Internal audit reports a finding once. The ATR tracks it until the risk has been dealt with.

Audit committees use it to see which findings are open, late or closed, and on what evidence. US teams call this issue or remediation tracking; in India, ATR is standard.

A good ATR gives each finding one owner, an agreed action, a target date and the evidence that the action was taken. Internal audit checks that evidence before closing the item.

Without one, the same finding often comes back in next year's audit.

An example

An audit finds that changes to vendor bank details are not verified. Management agrees to call the vendor on a known number before any change is saved. The ATR names the AP lead as owner and sets a date. At the next review, internal audit checks the call records and closes the item.

How IRAME handles it

IRAME's ControlDesk includes an action taken report module. In Action Hub, IRA, IRAME's AI auditor, routes every exception and finding to an owner and tracks it to closure. Evidence from physical work, such as a site visit, is uploaded against the item.
